Form I-589, Application for Asylum and for Withholding of Removal,
signed and completed. This form and accompanying instructions can
be found at http:// www .uscis .gov /i‑589.
Two additional copies of the form and all supporting evidence
If the applicant’s spouse or children are in the United States and included
as derivatives, the applicant must submit:
One additional copy of the completed asylum application for each
family member included in the application
Documents establishing the family relationship for each family
member
Copies of a marriage certicate and copies of proof of termi‑
nation of any prior marriage
For unmarried children under age 21, copies of each child’s
birth certicate
If the applicant cannot obtain these documents, he or she must
provide an afdavit from at least one person for each event.
