Use the following form if you have a focus group assess your case. You may change this form, as necessary, depending upon the size of the focus group, the type of focus group, and the specific facts of your case.

FOCUS GROUP QUESTIONNAIRE

Please answer the following questions to the best of your ability after having evaluated all positions and discussions regarding the case that you have been presented:

  1. How would you summarize what has happened in this case?

  2. Who do you believe is responsible for the plaintiff’s injuries?

  3. If you believe that more than one person is responsible for the plaintiff’s injuries, assign a percentage of responsibility to each as follows. If you believe that one or more of the parties are not responsible, simply assign zero percent.
