AUTHORIZATION FOR RELEASE OF MILITARY RECORDS

TO CUSTODIAN OF MILITARY RECORDS:

I, [name of person authorizing release], authorize [name of person/firm authorized to obtain records] or any of [his/her/its] representatives to obtain all information pertaining to my military service in the [specify branch of the Armed Forces]. Such information includes, but is not limited to, all records pertaining to my military service, disciplinary actions, training, employment, compensation, medical benefits, counseling, discharge, retirement, pension, etc., and all information I have, or anyone else has, disclosed pertaining to my military service. I request that you deliver such records at their request and at their expense, upon presentation of this authorization or a photocopy thereof.

This authorization is to be used in connection with the above-captioned lawsuit filed by me. This authorization is valid until [indicate a specific date, event, or condition].
