Annual Performance Evaluation
POSITION TITLE: Executive Director
ACCOUNTABLE TO: Board of Directors
BASIC FUNCTIONS: To manage and lead the operations of Interfaith. To create and maintain optimum relationships with
and between staff and volunteers within the agency as well as within the community.
INSTRUCTIONS: You, as a board member, are being asked to give your input regarding the performance of our executive
director for the year 2016. Please rate his performance on all nine principal activities by entering a number from 1 (being
the poorest performance) to 10 (reecting excellent performance) in the blank after each statement. Your comments are
also welcome at the end of the form. If you have no opinion on a statement, please enter “NA in the blank. Please send
this completed form to Lynn or bring the completed evaluation to IAM and have it placed in Lynn’s mailbox at IAM not later
than Friday, November 4.
PRINCIPAL ACTIVITIES:
1. Hires and evaluates reporting staff and is responsible for the selection and evaluation of all other paid staff. _________
2. Orients new members of the Board to agency, particularly its mission, goals and objectives. _________
3. Attends Board meetings and reports on agency operations, trends and activities in the community or elsewhere that
may affect agency operations or policy; attends meetings of the following Board Committees: Executive and Finance, to
offer counsel and advice; attends other Board Subcommittee meetings and maintains liaison by delegating administrative
support to a staff member. _________
4. Develops, in consultation with the Executive Committee and in coordination with other key staff members, agency goals
for Board approval. Works with key staff in the development of agency objectives. _________
5. Collaborates with the Treasurer and Finance Committee to develop an Annual Budget and nancial controls for Board
approval. _________
6. Maintains an ongoing relationship with other agencies and community groups to minimize duplication of services and
to seek out opportunities for collaborative resources and efforts. _________
7. Seeks out additional funding for Interfaith in cooperation with various subcommittees and the Board of Directors. _________
8. Approves all public relations, marketing, promotion and fundraising plans. _________
9. Keeps up-to-date on trends, legislation, etc. that affect Interfaith as well as managerial techniques and trends by at-
tendance at Rotary meetings, Chamber events, professional and/or association conferences, seminars, etc. _________
