Following are major Indiana health insurance plans, including indemnity plans for traditional fee-for-service health insurance: preferred-provider organizations (PPOs) or health-maintenance organizations (HMOs) offering managed care through a network of physicians and hospitals; and exclusive-provider organizations (EPOs) or point-of-service (POS) plans with aspects of both PPOs and HMOs. Aetna, Fort Wayne, is a POS, PPO and indemnity company for insured and self-insured clients. Aetna provides services to more than 302,000 Indiana residents, using some 5,864 physicians and 104 hospitals. 260/496-5400

Anthem Blue Cross and Blue Shield, a subsidiary of WellPoint Inc., Indianapolis, has HMO, PPO and POS plans serving 2,113,159 million members in Indiana.
