I. Introduction

On __________, 20___, the Board of Aldermen [or City Council] adopted Resolution No. ______ (attached as Exhibit A), which established:

The Board of Aldermen [or City Council] hereby declares its intent to annex a certain part of the unincorporated area of the County of __________ described in Exhibit A, attached and incorporated by reference in this Resolution (the "Annexation Area"), that the Annexation Area is contiguous to the existing limits of the City, and that the length of the contiguous boundary common to the existing City limit and the Annexation Area is at least 15% of the length of the perimeter of the Annexation Area and, accordingly, the boundaries of the Annexation Area comply with and satisfy the condition precedent set forth in subdivision 1(1) of the Annexation Statute.

This Plan of Intent provides an analysis of a proposed annexation area, the legal description of which is attached hereto as Exhibit B and is incorporated by reference as though set out in full herein.

II. Summary of Proposed Annexation Area

The proposed annexation area is approximately ______ square miles and has an estimated population of _________ people. A map of the proposed annexation area is provided in Figure 1, below. The existing land use within the proposed annexation area is a mix of __________ uses. This area, under the jurisdiction of __________ County, is zoned primarily as __________. The topography varies throughout.

The proposed annexation area is served by the __________ Public School District. No existing school facilities are located in the proposed annexation area.

III. Major Services Provided by the City and Schedule of Providing Services

The City of __________ currently provides a wide range of services to citizens residing within the present corporate limits. The City of __________ plans to provide all of the following services to the residents of the proposed annexation area as of the effective date of annexation, unless otherwise noted below.

A. Fire Department

Fire protection services within the City are currently provided by the City's Fire Department. Ambulance services are currently provided within the City's existing limits by the __________ Ambulance District. Fire protection and emergency services will be provided in full, as of the effective date of the annexation.

B. Police Department

The __________ Police Department has ______ full-time and ______ part-time officers. The Department utilizes ______ marked patrol vehicles, each of which is equipped with state-of-the-art radar and video recording apparatus.
