Comes now Plaintiff(s), by counsel, and in accordance with the provisions of § 537.095, RSMo, moves this Court to enter its order approving the proposed settlement for the following reasons and on the following terms:

1. Plaintiff has filed suit against Defendant for the wrongful death of __________, arising out of and as a result of __________.

2. Plaintiff has investigated the facts and circumstances of the occurrence and of the death of Decedent and of the potential elements of damage as set forth in § 537.090, RSMo.
