This Indenture witnesses that, in consideration of the sum of $ __________, the receipt of which is hereby acknowledged, for myself and for my heirs, personal representatives, and assigns, I do hereby release and forever discharge __________ from all claims, demands, costs, expenses, loss of services, actions, and causes of action arising from any act or occurrence up to the present time, and particularly because of all personal injury, disability, property damage, loss suffered, or damages of any kind sustained, and from any liability for contribution or apportionment of fault, arising out of or in any way pertaining to the injuries sustained by __________ and the death of __________, these acts and omissions to include, but not being limited to, those set forth in the petition filed by Plaintiff.
