AN ORDINANCE PROVIDING FOR THE ANNEXATION OF UNINCORPORATED TERRITORY AND THE EXTENSION OF THE CORPORATE LIMITS OF THE CITY OF __________, MISSOURI, ALL PURSUANT TO SECTION 71.015 OF THE REVISED STATUTES OF MISSOURI, AS AMENDED.

Whereas, the Board of Aldermen [or City Council] of the City of __________, Missouri (hereinafter "City") has determined that certain land located in the County of __________ is contiguous to the existing limits of the City and that the length of the contiguous boundary common to the existing City limit and the aforesaid area is at least 15% of the length of the perimeter of the area (the "Initial Determination"); and
