Health Care Directive

I, John Doe, do hereby make, publish, and declare this as my Health Care Directive, and if, and to the extent, any prior Directive by me is inconsistent with this Directive, I do hereby revoke any prior Directive made by me at any time.

I have the primary right to make my own decisions concerning medical treatment, even to the extent of refusing the initiation of life-sustaining treatment or demanding its withdrawal.

If I am incapacitated or otherwise unable to communicate my decisions regarding medical treatment, in the absence of “clear and convincing evidence” to the contrary, health care providers may be required to initiate or be prohibited from withdrawing medical treatment to sustain my life, unless such medical treatment would be clearly medically futile.

By this Directive, I intend to provide “clear and convincing evidence” to health care professionals, health care facilities, family members, and friends of my wishes regarding medical treatment if I am incapacitated or otherwise unable to communicate such on my own behalf.

If I am at any time unable for any reason to communicate my desires regarding medical treatment, the following provisions shall control:
