Assignment

I, _______________, the undersigned, have created a revocable living trust in accordance with a trust agreement by and between myself as “Grantor” and myself as “Trustee” under the trust agreement.

The trust was created and property transferred for the purpose, among others, of avoiding probate administration, at the time of my death, of the assets of the trust.

For that reason, I wish to transfer to the trust all of my tangible personal property listed on Schedule A, attached hereto, or any interest therein, including, but not limited to:

• household furniture and furnishings;

• appliances;

• equipment;

• antiques;

• works of art;

• china;

• silverware;

• glassware;

• books;

• jewelry;
