C. (§13.5) Petition for Guardianship

The contents of a petition are provided for in § 475.060, RSMo 2000. The petition must state the following:

· The name, age, domicile, actual place of residence, and post office address of the respondent if known or, if any of these facts are unknown, a statement of the efforts to ascertain the unknown facts

· The estimated value of the respondent’s real and personal property

· If the respondent has no domicile or place of residence in Missouri, the county in which the respondent’s property or a major part of it is located

· The names and addresses of the respondent’s parents and whether they are living or dead

· The name and address of the respondent’s spouse, if any, and the names, ages, and addresses of all living children of the respondent

· The name and address of the person having custody of the respondent

· The name
