Section 577.041.1, RSMo Supp. 2012, requires the officer to notify the person:

• the reasons for the officer requesting a person to submit to a chemical test;

• that evidence of the refusal to take the test may be used against the person and

• that refusal to take the test will result in the revocation of the person’s license.

If a person refuses to submit to a chemical test, the law enforcement officer is required to serve the person with the notice of the license revocation and the notice of the person’s right to file a petition for review to challenge the license revocation. Section 577.041.1.
