AUTHORIZATION FOR RELEASE OF SCHOLASTIC RECORDS

TO ALL CURRENT AND FORMER EDUCATIONAL INSTITUTIONS ATTENDED BY [NAME OF PERSON AUTHORIZING RELEASE]:

I, [name of person authorizing release], authorize [name of person/firm authorized to obtain records] or any of [his/her/its] representatives to obtain all information pertaining to my education, including, but not limited to, all test scores, grades received in all areas of study, counseling records, conference reports, disciplinary actions, attendance records, evaluation records and reports, statements or evaluations made by teachers or administrators, any medical testing performed at this facility, statements or notations made by the school nurse, and any other records pertaining to my education at your institution. I request that you deliver such records at their request and at their expense, upon presentation of this authorization or a photocopy thereof.
