In accordance with Supreme Court Rule 214, we request that you provide at our office for inspection and copying all documents of any kind or nature including, without limitation, correspondence, memoranda, notes, writings, drawings, graphs, charts, photographs, phone records and other data computations from which information can be obtained, and any and all copies thereof within your possession, custody or control, or within the possession, custody or control of your agents, servants or employees, relating to, connected with or resulting from the following:

(1) If a party has given a statement, oral or written or court reported, signed or unsigned, to some person or entity other than his or her attorney or insurer, said statement shall be produced by the person or entity to whom it was given or transferred.
