AGAINST DEFENDANT FOR CONDUCTING

IMPROPER TESTS ON PLAINTIFF

The Plaintiff, __________, by and through his attorneys, hereby requests that this honorable court impose sanctions on the Defendant for having subjected the Plaintiff to unnecessary, unauthorized and potentially harmful physical testing, and that this court further restrain Defendant from further physical testing of the Plaintiff, for the following reasons:

1. The Defendant did issue to the Plaintiff a Notice to Appear for a Physical Examination on _____ 20__ at the clinic of __________ located at ______.
