(g) Prior work experience of said expert, including:

(1) Employers for the past 10 years;

(2) Title or position held;

(3) Duties and responsibilities;

(4) Special training or experience; and

(5) Any other experience which said expert will rely upon in formulating an opinion relative to this litigation;

(h) State any and all publications by or contributed to by said expert, and as to each publication, state:

(1) Title, authors and publishers;

(2) Date of publication;

(3) Content and subject; and

(4) Whether any of the above described publications will be relied upon by said expert in formulating opinions in this litigation. If so, please identify which publications will be so utilized.
