Landlord filed an application for high-rent/high-income deregulation of tenant's rent-stabilized apartment in 2010. The DRA dismissed landlord's application because the Income Certification Form (ICF) used by landlord wasn't an agency-approved form and contained a number of defects and errors.

Landlord appealed, and the DHCR reopened the case. Landlord downloaded the DHCR's ICF form from the agency's Web site and then converted it to a "fillable" form using Adobe Professional software. This caused a few minor typographical errors and font size changes to appear...
