A. Maryland Uniform Child Custody Jurisdiction and Enforcement Act⁷²

In any custody proceeding (initial proceeding or a modification), each party's initial pleading (or accompanying affidavit) must state the child's present address, the places where the child has lived for the past five years, and the names and addresses of the persons with whom the child has lived during that period.⁷³ In addition, each party must declare under oath whether:
