The Petitioner, JANE SMITH, is the maternal grandmother of TIM SMITH, the minor child(ren).

The Petitioner alleges that there is a pre-exisiting relationship between the petitioner and the child(ren) that has engendered a bond such that it is in the best interest of the child(ren) that the court order reasonable visitation.

And one of the following is true:

[ ] That the parents are currently living apart on a permanent or indefinite basis.
[ ] That one of the parents has been absent for more than one month and the remaining parent does not know the whereabouts of the absent parent.
[ ] That one of the parents has joined in this petition.
[ ] That the child is not residing with either parent.
[ ] That the child has been adopted by a step parent.

It is in the best interest of the minor child to maintain the existing close personal relationship with the Petitioner.

A completed Declaration Under the Uniform Child Custody Jurisdiction and Enforcement Act is incorporated herein.
