ELECTRONIC FORMS: INSTRUCTIONS AND TABLE
The forms in the appendices follow a few conventions the practitioner should find helpful. The language enclosed in boldface brackets ([example]) may be optional, alternative, suggested, or provided by way of example, depending upon the context in which it is used. Some items such as names, dates, and some dollar amounts that may not be enclosed in brackets are also clearly intended only as examples. The practitioner must always exercise independent professional judgment when making use of any of the information or forms contained in this book.
The print edition of this handbook includes a CD-Rom containing electronic forms. The same forms are embedded in the electronic version of this book and can be accessed directly from this table of forms page, as described below. Unlike the forms on the CD-Rom, the electronic forms in this handbook are licensed for use on a single personal computer only.
Electronic forms, whether included in the print edition or the electronic (PDF) edition, differ from the sample forms found in the appendix of the printed book, which have been printed single-spaced and truncated to conserve space. Electronic forms include the captions at the top of the forms and the certificates of mailing at the bottom and they are double-spaced, and so comply with Rule 1:16 of the Virginia Supreme Court, which requires that documents filed in court be double-spaced. They may be copied for use within the law firm of the person who purchased the associated book. Forms may be modified as necessary to suit clients' needs.
