§ 3.4 • JURY QUESTIONNAIRES

Colorado

General. On or before the first day of juror service, each juror must complete a questionnaire, declare that the information in the questionnaire is accurate, and sign the questionnaire. C.R.S. § 13-71-115(1).
Information in the Questionnaire. The questionnaire shall request the following information about jurors: their name, gender, date of birth, age, residence, and marital status; the number and ages of the juror's children; the juror's educational level and occupation; whether the juror is regularly employed, self-employed, or unemployed; the occupation of the juror's spouse; any previous juror service; any present or past involvement as a party or witness in a civil or criminal proceeding; and such other information as may be requested by the jury commissioner. C.R.S. § 13-71-115(1).
