EXHIBIT 15E • SAMPLE SIMPLE COOPERATIVE MEMBERSHIP APPLICATION {__________ CO-OPERATIVE} MEMBERSHIP APPLICATION

The undersigned applies for membership in the {__________ Co-operative} and agrees to abide by the Articles of Incorporation, the Bylaws and polices, rules and regulations of the Cooperative, which are in effect now or in the future.

The undersigned specifically acknowledge and agree to the following provision of the Bylaws:

2.8 CONSENT TO TAX TREATMENT
Each individual who hereafter applies for and is accepted as a member in this cooperative on or after (Date) , which shall be the effective date of this bylaw, who continues as a member after such date shall by such act alone consent that the amounts of any distributions with respect to his or her patronage occurring on or after (Date) , which are made in qualified written notices of allocation (as defined in 26 U.S.C. 1388) and per unit retain allocations which are received by him or her from the Co-op, will be taken into account by the member at their stated dollar
