When a physician indicates that he or she will need visa sponsorship as part of his or her placement, your immigration attorney normally will need to know certain details regarding the employment opportunity. To help you determine what opportunities will and will not work for a particular physician, it will help us to know the following:

1. Who will be the actual employer of the physician (a group, the hospital or facility, a staffing/locums employer, the physician will have a solo practice, etc.)? ___________________________________________________________
