Papers
The list below describes some of the information collected from and signed by the birth parents before the
baby is born and which may be filed with the initial court filing to notify the court of the circumstances
surrounding the proposed adoption:
• A disclosure form identifying the general rights of the birth parents under applicable state law
and a statement indicating that the birth parents have received such disclosure. This document,
known in Florida as the Adoption Disclosure Form, is the “bible” of the parties’ rights and should
be read and discussed very carefully.
• A genetic history and background information questionnaire detailing the background of the
birth mother (and, where known and available, the birth father), including the medical histo-
ries of all children, parents, brothers, sisters, grandparents, and other relatives. The information
gathered is intended to reveal to the prospective adoptive family the history of the birth parent(s)
from ethnic, racial, medical, educational, occupational, legal, and social perspectives. This will
also include information such as allergies, medical conditions, addictions, social, emotional, psy-
chiatric, and similar conditions. It may also include the goals of the birth parent(s), their reason
for contemplating adoption, recent employment, arrests, and other information which may be
helpful for the family to know as the child grows up.
• A disclosure as to whether the birth mother is married or was cohabiting at the time she became
pregnant and any and all details surrounding this issue should be documented. In this regard,
