Sample Instruction

An employee, officer, agent, or manager of a corporation or other business entity who approved of, directed, actively participated in, or cooperated in the negligence of the corporation or entity is personally responsible for any damages caused by such negligence.

You are instructed to answer the following questions. You must all agree on your answers to each question and you must all sign the completed form on the signature lines:
