D. (§9.10) Contents of the Petition—Guardianship

The petition must state:

· the respondent’s name;

· the respondent’s age;

· the respondent’s domicile;

· the respondent’s actual place of residence;

· the respondent’s post office address at the time of filing, and for the prior three years before filing the petition;

· the estimated value of the respondent’s real and personal property; and

· if there is no domicile or place of residence in this state, the county in which the property is located.

· The respondent’s parents, spouse, and children, or, if none, the respondent’s siblings, and children of deceased siblings
