§ 34.1.12 Disputes Between Provider and Contractor. A contractor or provider who has a dispute with AHCCCS or a contractor over claims payment, sanctions, or reinsurance must specify in detail the factual and legal basis for the claim and the relief requested. A.A.C. R 9-34-404.
