2. [§ 6.22] Required contents and exhibits of initial foreclosure filing

A Complaint to Foreclose or an Order to Docket to foreclose a mortgage or deed of trust on residential property shall include, or be accompanied, by the following:

1. If applicable, the license number of the mortgage originator and the mortgage lender;
2. An affidavit stating the date on which the default occurred and the nature of the default, and, if applicable, that a notice of intent to foreclose was sent, the date the notice was sent, and that the contents of the notice were accurate at the time it was sent;
3. The original or a certified copy of the mortgage or deed of trust;
