19.6 Record-Keeping. All mental health professionals have a duty to prepare and maintain professional records adequate to document services and to facilitate continuity of services.26 The failure to maintain adequate records can result in administrative disciplinary action against the professional and may form the basis for civil remedies in a civil action.
