19-06 INSTRUCTIONS FOR MAKING APPLICATION FOR CONDITIONAL USE PERMIT

1. The applicant (owner or authorized agent) shall appear at the Department of Planning and Development, [address], and complete the required application forms. [NOTE: In some municipalities, these forms may be downloaded and filed by electronic mail or other submittal process.]

2. The applicants shall submit, as part of the application, a cover letter setting forth the request. The letter should include a statement as to the intent of the use and should include such information as is necessary to inform the Commission of the issue. Any known issue which will require a variance or waiver should be outlined in the letter.
