NOW ON THIS DAY, this case comes before the Court for consideration, with Plaintiff, [plaintiff name] (hereinafter "Payee Parent"), appearing by and through [his/her] attorney, [plaintiff attorney name]. From the pleadings and other matters properly before the Court, the Court FINDS and ORDERS:
