Defendant moves the Court for a change of venue pursuant to Ark. Code Ann. §§ 16-88-201 et seq., and [he/she] states as follows:

1. Defendant is charged with the offense of [offense].

2. The minds of the inhabitants of this County are so prejudiced against Defendant that a fair and impartial trial cannot be held in this County.

3. Attached hereto in support of such allegations are the affidavits of [name of affiant] and [name of affiant], two credible persons who are qualified electors and actual residents of this County and who are not related to Defendant in any way, which state that they are of a mind that there is sufficient prejudice existing throughout the county that Defendant cannot get a fair trial here.
