§ 37.3.8.2 Disclosure Requirements. In order to constitute a disclosure protected by the statute, the disclosure must be in writing and contain (1) the date of the disclosure, (2) the name of the employee making the disclosure, (3) the nature of the alleged violation of law, mismanagement, gross waste of money or abuse of authority, and (4) the date or range of dates the alleged violations occurred.
